I confirm that I have a physical disability, medical condition or injury which means that I am unable to walk or have difficulty in walking, as per UK law.

I confirm that, as far as I am aware, I do not have any condition, impairment or medication that would impair my ability to safely operate equipment provided to me by Shopmobility or render my use of such equipment as a danger to others. I confirm I will not drive them under the influence of alcoholic drink or illicit drugs.

I accept that any electrical vehicle loaned to me must be driven with due care and attention and manoeuvred at the appropriate speed, giving priority to pedestrians and must not be driven on the road. I agree to keep my arms and legs within the frame of the scooter when in moton to avoid injury. I confirm that I will not use the scooter outdoors in wet rainy conditions, especially in heavy rain.

I will report the details of any accident, where damage or injury is caused to a person or property, to Shopmobility staff as soon as possible.

I accept that no passengers of any age are to be carried on any of the vehicles loaned, other than user. I confirm I will not place shopping bags on the handlebars, or excessive baggage on the vehicle.

I accept that the vehicle is for my sole use and is not to be used by any other person and that I, including goods that I am carrying, must not exceed the weight limit of the designated vehicle. Standard vehicle - 21.6 stones / 137kg. Large vehicle – 35 stones / 220kg.

I accept that by using the equipment loaned to me I shall be liable for any damage, loss or repairs that may occur because of my negligence or misconduct.

Our equipment is free to use but you may be charged if is returned damaged. If the scooter goes missing or is damaged beyond repair, you may be charged the cost of repair or replacement. If the scooter is damaged and repairable – you will be charged 50% of the repair cost. If the scooter goes missing or is damaged outside of the one mile radius limit you will be responsible for all charges. I agree to abide by all Shopmobility’s rules and regulations.
